Is う not used here in the passive form most of the time? If you are telling someone to do something, or someone is telling you to do something, then whoever is being told is at a disadvantage, or am I misunderstanding?

Make sure you check out the model sentences - lays it out pretty clearly, I think (hope!). The receiver is not at a 'disadvantage' per se - if a doctor tells you to do something, like take some medicine, I wouldn't think that so terrible that you'd use the passive to register your dissatisfaction with what's going on.
