Hmm - I haven't been able to replicate this yet. I tried the same question type with the same kanji - the first time, I picked on/on (wrong), then in the review, I tried kun/kun (correct).
Can you recall any differences from when you did it?
Also, if you can, tap the ? button the next time it happens and send the quiz data - that may shed a light on what is happening.
for this case, my quiz sequence, i think i had 2 questions wrong. this was the first question to be wrong and marked to be redone. Other than that, i don't remember. I'll gather the specific information in more detail next time.
