Some more info! Some kanji in My Kanji list are automatically being marked as known.

These 6 aren't in in any schedule or lesson, I made sure I marked all not in schedule/lesson as unknown before my previous post, and yet these are now Known after they showed up in a quiz.
Not sure if some of these are new to the My Kanji list, though.
edit: definitely being added, this is after two quizzes with the same issue
Sorry for the delay! Things are slower for me on the weekend.
On the My Kanji list, tap Tools. Then try marking them as unknown. That *should* pull them out. If it doesn't, let me know!
Currently, the "My___" lists are all the terms that have mastery data on them in one way or another. I'll consider adjusting that in the future, but that's the way they are marked now.
So, I checked on 吋 - and it was marked in your system as being seen inyour quiz history. In fact, it says that it was in an onyomi-vector'ed question at some point. Now, it was not marked as known, in the sense that you, for example, marked it with a lightbulb.
I checked 鯵 as well, same exact state. Regarding the second, this is the "newest", so we'll probably be able to piece together the info with that one. So, did you see it appear in a question during those last two quizzes? Maybe as an extra kanji in one of the question modes?
No worries, I was just adding new info I found, there is certainly no hurry on this :)
I am marking them as Unknown in the Japanese Index, because in Tools in the My Kanji list all of them show up and I cannot filter only by the ones I want out. However, doing this is not removing them from my Kanji, and in fact, new ones keep being added to My Kanji. I can try to reset mastery to 0% to all of those and see if that will take them out?
Regarding 吋 and 鯵, for me, they appear as Known (the ligthbulb is on), but I don't think I did that manually. And yes: they both appeared in an on-yomi question in a quiz, specifically, in a "Find the impostor" question tipe. If it helps, the next time something like it happens, I can send an in-quiz report!
