This is often used to show the extremity of a situation; while the listener might think of A, it's actually far worse.

Example:
しくて、するどころか、テレビをるひまさえない
I'm so busy I don't even have time to watch TV, let alone go on a trip.

"B, not to mention A"; "not just A, but also B"
のAライセンスどころか、っている
She doesn't just have a class A license, she has a pilot's license too.

(The latter usage seems comparable in meaning to はもちろん)

Contruction is the same:
Plain form verb + どころか
Noun + どころか
い adjective + どころか
な adjective + どころか

しくて、するどころか、テレビをるひまさえない。
のAライセンスどころか、っている。

I think the definitions are one in the same, so I simply extended the original definition out. Let me know if you think it should be altered to clarify it some. If you check out the model sentences, they're pretty close to the ones you gave :)
