In the sense of "A is already sufficient, but in addition we have B". E.g. "not only the food is bad, but the service is horrible", "not only does he speak 8 languages, but he also plays the violin".
The difference between "~ばかりでなく…も" is that "~ばかりか" expresses the feelings of surprise or exclamation.
