Why is this listed as just "必要" instead of "必要はない"? "必要" by itself doesn't line up with the listed meaning or structure.

The second meaning (slightly further down the page) is 必要がある, so 必要 is the common element that they both share :)

What about Noun + が必要だ formations like this one? for example:
お金が必要です。
I need money.
okane ga hitsuyou desu.

I didn't put this in originally because it is not a unique expression, just a noun + が  + adjective. Since 必要 is up here, though, it wouldn't hurt to add it!

I think some of the example sentences are mixed up. Like,
「この書類にあなたのサインが必要です。」 should be under meaning 1;
「新しいプロジェクトについて上司と相談する必要がある。」should be meaning 3;
and「修理の必要があります。」should, I think, also be meaning 3, though the construction is missing the use of noun + の + 必要がある.
