Considering the situation, conditions of the time and social rules, speaker expresses notion of “must by all means,” or “own feeling that…must be done.” Formal expression.
It helps to think of this grammar as two parts. The first one being negative A, (ずに) / (ないで) and the second one being B, the negative of 済む (すまない).
So, when A is negative (not done) B won't do/end/suffice.
In other words, it must be done, otherwise B cannot occur because A wasn't done first.
